Case Manager - care planning (remote)

Silverline Sable Community Partners is a Colorado Springs charity working with residents on basic needs and opportunity. We are hiring a Case Manager to take on care planning. You would join a large organisation of about 223 people, working mostly from home. What you will do • Connect clients with partner agencies. • Help with applications and appointments for donor records. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with donor records. Working arrangement • Full-time, Monday to Friday. • Fully remote, with a monthly team day in Colorado Springs. • We will tell you what to expect at each step of the hiring process.
