Case Manager - care planning (remote)

We are growing our team at Stonegate Glen Civic Services and adding a Case Manager whose main responsibility will be care planning. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for case files. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with case files. Working arrangement • Full-time, with a steady weekday schedule. • Remote-friendly, with a monthly team day in Albuquerque. • We will tell you what to expect at each step of the hiring process.
