Case Manager - care planning (remote)

We are growing our team at Highbridge Vale Community Partners Group and adding a Case Manager whose main responsibility will be care planning. You would join a small organisation of about 191 people, working mostly from home. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for grant reporting. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with grant reporting. Working arrangement • Part-time, about 24 hours a week, mornings. • Fully remote, with an occasional day on site in Tucson. • We reply to every applicant.
