Case Manager - care planning (remote)

Bluepeak Juniper Health runs clinics and patient services around Baltimore. We are hiring a Case Manager to take on care planning. Continuing education is supported. What you will do • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for insurance authorisations. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with insurance authorisations. Working arrangement • Part-time, about 28 hours a week, spread across three days. • Fully remote, with quarterly visits in Baltimore. • We reply to every applicant.
