Case Manager - care planning (remote)

Bluepeak Iron Community Partners is looking for a Case Manager. The job centres on care planning for benefits enrolment, and it would suit someone who likes steady, practical work. We are small enough to know everyone's name. What you will do • Connect clients with partner agencies. • Help with applications and appointments for benefits enrolment. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with benefits enrolment. • Bilingual English and Spanish. Working arrangement • Part-time, about 16 hours a week, spread across three days. • Remote-friendly, with an occasional day on site in Raleigh. • We will tell you what to expect at each step of the hiring process.
