Case Manager - care planning (remote)

Harborlight Glen Civic Services provides shared administrative services to local governments around Salt Lake City, and we need a Case Manager to lead our care planning. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for permits. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with permits. Working arrangement • Full-time, with a steady weekday schedule. • Remote-friendly, with quarterly visits in Salt Lake City. • We will tell you what to expect at each step of the hiring process.
