Case Manager - care planning (remote)

Northwind Clear Care Services supports families and individuals in Des Moines with personal care and coordination. We are hiring a Case Manager to take on care planning. You would join a small organisation of about 127 people, working mostly from home. What you will do • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Help with applications and appointments for licensing. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with licensing. Working arrangement • Contract for four months, with clear deliverables from the first week. • Remote-friendly, with an occasional day on site in Des Moines. • We will tell you what to expect at each step of the hiring process.
