Case Manager - care planning (remote)

Summit Kestrel Housing is a nonprofit housing provider in Madison. We are hiring a Case Manager to take on care planning. You would join a small organisation of about 60 people, working mostly from home. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for lease compliance. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with lease compliance. Working arrangement • Contract for three months, with a review at the end and a chance of extension. • Remote-friendly, with a monthly team day in Madison. • Applications are reviewed on a rolling basis.
