Case Manager - care planning (remote)

Northwind Glen Care Services provides in-home and community support in Philadelphia. We are hiring a Case Manager to take on care planning. You would join a small organisation of about 24 people, working mostly from home. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for licensing. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with licensing. • Bilingual English and Spanish. Working arrangement • Full-time, with a steady weekday schedule. • Remote-friendly, with an occasional day on site in Philadelphia. • We will tell you what to expect at each step of the hiring process.
