Case Manager - care planning (remote)

Cedar Drift Health Group is looking for a Case Manager. The job centres on care planning for insurance authorisations, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for insurance authorisations. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with insurance authorisations. Working arrangement • Contract for three months, with clear deliverables from the first week. • Remote-friendly, with an occasional day on site in Honolulu. • Applications are reviewed on a rolling basis.
