Case Manager - care planning (remote)

We are growing our team at Redstone Corner Learning Group and adding a Case Manager whose main responsibility will be care planning. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for student accounts. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with student accounts. Working arrangement • Full-time, Monday to Friday. • Fully remote, with quarterly visits in Raleigh. • We will tell you what to expect at each step of the hiring process.
