Case Manager - care planning (remote)

Stonegate Iron Property Management is a Chicago property manager with leasing, maintenance and resident services. We are hiring a Case Manager to take on care planning. Residents are neighbours, not numbers. What you will do • Help with applications and appointments for maintenance requests. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with maintenance requests. • Bilingual English and Spanish. Working arrangement • Full-time, with a steady weekday schedule. • Fully remote, with a monthly team day in Chicago. • We reply to every applicant.
