Case Manager - care planning (remote)

Amberfield Harbor Care Services provides in-home and community support in Toledo. We are hiring a Case Manager to take on care planning. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Help with applications and appointments for visit scheduling. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with visit scheduling. Working arrangement • Full-time, Monday to Friday. • Remote-friendly, with an occasional day on site in Toledo. • We will tell you what to expect at each step of the hiring process.
