Case Manager - care planning (remote)

Silverline Mill Care Services supports families and individuals in Detroit with personal care and coordination. We are hiring a Case Manager to take on care planning. You would join a small organisation of about 14 people, working mostly from home. What you will do • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for care plans. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with care plans. Working arrangement • Contract for six months, with a review at the end and a chance of extension. • Fully remote, with a monthly team day in Detroit. • We will tell you what to expect at each step of the hiring process.
