Case Manager - care planning (remote)

Stonegate Juniper Health Group is looking for a Case Manager. The job centres on care planning for electronic records, and it would suit someone who likes steady, practical work. You would join a small organisation of about 20 people, working mostly from home. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for electronic records. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with electronic records. • Bilingual English and Spanish. Working arrangement • Part-time, about 20 hours a week, mornings. • Fully remote, with quarterly visits in Oakland. • Applications are reviewed on a rolling basis.
