Case Manager - care planning (remote)

Foxglove Quill Care Services is looking for a Case Manager. The job centres on care planning for client records, and it would suit someone who likes steady, practical work. We train and promote from within. What you will do • Follow up regularly and update records accurately. • Help with applications and appointments for client records. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with client records. Working arrangement • Full-time, on a regular day shift. • Fully remote, with a monthly team day in Kansas City. • Applications are reviewed on a rolling basis.
