Case Manager - care planning (remote)

Silverline Orchard Care Services supports families and individuals in Omaha with personal care and coordination, and we need a Case Manager to lead our care planning. Teams support each other, especially on hard days. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for licensing. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with licensing. • Bilingual English and Spanish. Working arrangement • Full-time, with a steady weekday schedule. • Fully remote, with a monthly team day in Omaha. • We will tell you what to expect at each step of the hiring process.
