Case Manager - care planning (remote)

Harborlight Junction Housing Partners is looking for a Case Manager. The job centres on care planning for rent assistance, and it would suit someone who likes steady, practical work. You would join a small organisation of about 109 people, working mostly from home. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for rent assistance. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with rent assistance. Working arrangement • Part-time, about 28 hours a week, spread across three days. • Fully remote, with a monthly team day in Louisville. • We reply to every applicant.
