Case Manager - care planning (remote)

Highbridge North Early Learning runs childcare centres in Savannah caring for infants, toddlers and preschoolers, and we need a Case Manager to lead our care planning. You would join a small organisation of about 14 people, working mostly from home. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for tuition and subsidies. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with tuition and subsidies. Working arrangement • Contract for eight months, with the possibility of becoming permanent. • Fully remote, with a monthly team day in Savannah. • Applications are reviewed on a rolling basis.
