Case Manager - care planning (remote)

Foxglove Lakeside Civic Services supports public agencies in Providence with budgeting, records and programmes, and we need a Case Manager to lead our care planning. Public service means patience, paperwork and pride in the outcome. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for budget submissions. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with budget submissions. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • Fully remote, with an occasional day on site in Providence. • We will tell you what to expect at each step of the hiring process.
