Case Manager - care planning (remote)

We are growing our team at Nova Juniper Health and adding a Case Manager whose main responsibility will be care planning. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for patient education. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with patient education. Working arrangement • Contract for three months, with the possibility of becoming permanent. • Remote-friendly, with a monthly team day in Milwaukee. • We reply to every applicant.
