Case Manager - care planning (remote)

Fernwood Amber Property Management Associates is looking for a Case Manager. The job centres on care planning for unit turnovers, and it would suit someone who likes steady, practical work. Maintenance and leasing staff share information every day. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for unit turnovers. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with unit turnovers. • Bilingual English and Spanish. Working arrangement • Part-time, about 28 hours a week, afternoons. • Fully remote, with quarterly visits in Boston. • Applications are reviewed on a rolling basis.
