Case Manager - care planning (remote)

We are growing our team at Silverline Vista Property Management and adding a Case Manager whose main responsibility will be care planning. Residents are neighbours, not numbers. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Help with applications and appointments for vendor invoices. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with vendor invoices. • Bilingual English and Spanish. Working arrangement • Part-time, about 28 hours a week, spread across three days. • Fully remote, with a monthly team day in Chicago. • We will tell you what to expect at each step of the hiring process.
