Case Manager - care planning (remote)

Fernwood Mill Care Services Group provides in-home and community support in Honolulu, and we need a Case Manager to lead our care planning. Teams support each other, especially on hard days. What you will do • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for licensing. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with licensing. • Bilingual English and Spanish. Working arrangement • Contract for six months, with clear deliverables from the first week. • Fully remote, with a monthly team day in Honolulu. • We will tell you what to expect at each step of the hiring process.
