Case Manager - care planning (remote)

Foxglove Sable Community Partners is a nonprofit in Kansas City supporting families with food, housing and job help. We are hiring a Case Manager to take on care planning. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for client intake. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with client intake. Working arrangement • Contract for four months, with the possibility of becoming permanent. • Fully remote, with an occasional day on site in Kansas City. • Applications are reviewed on a rolling basis.
