Case Manager - care planning (remote)

We are growing our team at Amberfield Meadow Housing and adding a Case Manager whose main responsibility will be care planning. You would join a large organisation of more than 1,000 people, working mostly from home. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for lease compliance. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with lease compliance. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • Fully remote, with an occasional day on site in Toledo. • Applications are reviewed on a rolling basis.
