Case Manager - care planning (remote)

Stonegate Oak Care Services provides in-home and community support in San Jose. We are hiring a Case Manager to take on care planning. You would join a small organisation of about 60 people, working mostly from home. What you will do • Help with applications and appointments for care plans. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with care plans. Working arrangement • Full-time, on a regular day shift. • Remote-friendly, with an occasional day on site in San Jose. • We will tell you what to expect at each step of the hiring process.
