Case Manager - care planning (remote)

Harborlight Yarrow Health is a healthcare provider in Albuquerque with clinics and support teams. We are hiring a Case Manager to take on care planning. You would join a small organisation of about 28 people, working mostly from home. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for referrals. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with referrals. Working arrangement • Contract for twelve months, with a review at the end and a chance of extension. • Remote-friendly, with quarterly visits in Albuquerque. • We will tell you what to expect at each step of the hiring process.
