Case Manager - care planning (remote)

Northwind Nest Community Partners is a nonprofit in Portland supporting families with food, housing and job help, and we need a Case Manager to lead our care planning. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for benefits enrolment. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with benefits enrolment. Working arrangement • Full-time, on a regular day shift. • Remote-friendly, with a monthly team day in Portland. • Applications are reviewed on a rolling basis.
