Case Manager - care planning (remote)

Harborlight Birch Care Services is a social-care provider serving older adults and people with disabilities around San Diego, and we need a Case Manager to lead our care planning. You would join a small organisation of about 124 people, working mostly from home. What you will do • Help with applications and appointments for care plans. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with care plans. • Bilingual English and Spanish. Working arrangement • Full-time, on a regular day shift. • Fully remote, with a monthly team day in San Diego. • We will tell you what to expect at each step of the hiring process.
