Case Manager - care planning (remote)

Harborlight Glen Health Partners is a healthcare provider in Madison with clinics and support teams. We are hiring a Case Manager to take on care planning. You would join a large organisation of more than 1,000 people, working mostly from home. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for insurance authorisations. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with insurance authorisations. • Bilingual English and Spanish. Working arrangement • Part-time, about 28 hours a week, spread across three days. • Fully remote, with an occasional day on site in Madison. • We reply to every applicant.
