Case Manager - care planning (remote)

Summit Cedar Health Group runs clinics and patient services around Tampa, and we need a Case Manager to lead our care planning. You would join a large organisation of about 597 people, working mostly from home. What you will do • Help with applications and appointments for electronic records. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with electronic records. Working arrangement • Full-time, with a steady weekday schedule. • Remote-friendly, with an occasional day on site in Tampa. • Applications are reviewed on a rolling basis.
