Case Manager - care planning (remote)

Fernwood Amber Property Management looks after rental homes across the Atlanta area. We are hiring a Case Manager to take on care planning. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for rent collection. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with rent collection. Working arrangement • Full-time, Monday to Friday. • Fully remote, with an occasional day on site in Atlanta. • Applications are reviewed on a rolling basis.
