Case Manager - care planning (remote)

Harborlight Cobalt Learning Group operates schools and learning programmes around Jacksonville, and we need a Case Manager to lead our care planning. Collaboration between teachers and staff is strong. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for school calendars. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with school calendars. • Bilingual English and Spanish. Working arrangement • Contract for six months, with clear deliverables from the first week. • Remote-friendly, with a monthly team day in Jacksonville. • Applications are reviewed on a rolling basis.
