Case Manager - care planning (remote)

Fernwood Kestrel Property Management looks after rental homes across the Des Moines area, and we need a Case Manager to lead our care planning. You would join a small organisation of about 27 people, working mostly from home. What you will do • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for vendor invoices. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with vendor invoices. • Bilingual English and Spanish. Working arrangement • Full-time, on a regular day shift. • Fully remote, with a monthly team day in Des Moines. • We reply to every applicant.
