Case Manager - care planning (remote)

Silverline Cobalt Housing Group is looking for a Case Manager. The job centres on care planning for waiting lists, and it would suit someone who likes steady, practical work. We value patience and follow-through. What you will do • Help with applications and appointments for waiting lists. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with waiting lists. • Bilingual English and Spanish. Working arrangement • Part-time, about 20 hours a week, afternoons. • Fully remote, with an occasional day on site in Jacksonville. • Applications are reviewed on a rolling basis.
