Case Manager - care planning (remote)

Silverline Linden Health Group is looking for a Case Manager. The job centres on care planning for patient education, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for patient education. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with patient education. Working arrangement • Part-time, about 24 hours a week, with flexible days. • Fully remote, with an occasional day on site in Little Rock. • We reply to every applicant.
