Case Manager - care planning (remote)

We are growing our team at Bluepeak Granite Community Partners Group and adding a Case Manager whose main responsibility will be care planning. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for client intake. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with client intake. Working arrangement • Full-time, Monday to Friday. • Fully remote, with a monthly team day in El Paso. • Applications are reviewed on a rolling basis.
