Case Manager - care planning (remote)

Summit Pine Housing is looking for a Case Manager. The job centres on care planning for waiting lists, and it would suit someone who likes steady, practical work. You would join a small organisation of about 16 people, working mostly from home. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for waiting lists. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with waiting lists. • Bilingual English and Spanish. Working arrangement • Part-time, about 24 hours a week, spread across three days. • Remote-friendly, with a monthly team day in Nashville. • Applications are reviewed on a rolling basis.
