Case Manager - care planning (remote)

Bluepeak Bright Community Partners runs community programmes across Denver. We are hiring a Case Manager to take on care planning. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for grant reporting. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with grant reporting. • Bilingual English and Spanish. Working arrangement • Part-time, about 16 hours a week, with flexible days. • Remote-friendly, with an occasional day on site in Denver. • We reply to every applicant.
