Case Manager - care planning (remote)

Nova Cross Civic Services Group supports public agencies in Chicago with budgeting, records and programmes, and we need a Case Manager to lead our care planning. You would work closely with colleagues in operations, support and management. What you will do • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for public records. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with public records. • Bilingual English and Spanish. Working arrangement • Full-time, on a regular day shift. • Fully remote, with quarterly visits in Chicago. • We will tell you what to expect at each step of the hiring process.
