Case Manager - care planning (remote)

Bluepeak Vale Community Partners Group runs community programmes across San Diego, and we need a Case Manager to lead our care planning. You would join a small organisation of about 30 people, working mostly from home. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for grant reporting. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with grant reporting. • Bilingual English and Spanish. Working arrangement • Part-time, about 24 hours a week, spread across three days. • Remote-friendly, with quarterly visits in San Diego. • Applications are reviewed on a rolling basis.
