Case Manager - care planning (remote)

Lumen Corner Early Learning is looking for a Case Manager. The job centres on care planning for enrolment, and it would suit someone who likes steady, practical work. You would join a small organisation of about 11 people, working mostly from home. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for enrolment. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with enrolment. Working arrangement • Full-time, Monday to Friday. • Remote-friendly, with a monthly team day in Virginia Beach. • We will tell you what to expect at each step of the hiring process.
