Case Manager - care planning (remote)

Cedar Keystone Health is a healthcare provider in Sacramento with clinics and support teams. We are hiring a Case Manager to take on care planning. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for referrals. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with referrals. Working arrangement • Contract for six months, with the possibility of becoming permanent. • Fully remote, with an occasional day on site in Sacramento. • We will tell you what to expect at each step of the hiring process.
