Case Manager - care planning (remote)

Stonegate Yarrow Health runs clinics and patient services around Sacramento. We are hiring a Case Manager to take on care planning. You would join a large organisation of more than 1,000 people, working mostly from home. What you will do • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for referrals. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with referrals. Working arrangement • Part-time, about 28 hours a week, spread across three days. • Fully remote, with a monthly team day in Sacramento. • We will tell you what to expect at each step of the hiring process.
