Case Manager - care planning (remote)

Greenridge Bright Community Partners is a nonprofit in Chicago supporting families with food, housing and job help. We are hiring a Case Manager to take on care planning. You would join a small organisation of about 37 people, working mostly from home. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for volunteer shifts. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with volunteer shifts. • Bilingual English and Spanish. Working arrangement • Part-time, about 24 hours a week, afternoons. • Fully remote, with an occasional day on site in Chicago. • Applications are reviewed on a rolling basis.
