Case Manager - care planning (remote)

Bluepeak Rowan Care Services supports families and individuals in Minneapolis with personal care and coordination, and we need a Case Manager to lead our care planning. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for care plans. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with care plans. Working arrangement • Part-time, about 28 hours a week, afternoons. • Remote-friendly, with quarterly visits in Minneapolis. • We will tell you what to expect at each step of the hiring process.
