Case Manager - care planning (remote)

We are growing our team at Redstone Loom Housing Group and adding a Case Manager whose main responsibility will be care planning. You would join a small organisation of about 112 people, working mostly from home. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for waiting lists. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with waiting lists. • Bilingual English and Spanish. Working arrangement • Full-time, on a regular day shift. • Fully remote, with an occasional day on site in Indianapolis. • We reply to every applicant.
