Case Manager - care planning (remote)

We are growing our team at Bluepeak Juniper Care Services and adding a Case Manager whose main responsibility will be care planning. You would join a large organisation of more than 1,000 people, working mostly from home. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for caregiver training. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with caregiver training. • Bilingual English and Spanish. Working arrangement • Contract for four months, with clear deliverables from the first week. • Remote-friendly, with an occasional day on site in Portland. • We will tell you what to expect at each step of the hiring process.
