Case Manager - care planning (remote)

We are growing our team at Summit Cobalt Property Management and adding a Case Manager whose main responsibility will be care planning. You would join a large organisation of about 943 people, working mostly from home. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for rent collection. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with rent collection. Working arrangement • Full-time, on a regular day shift. • Remote-friendly, with a monthly team day in San Antonio. • We will tell you what to expect at each step of the hiring process.
